For non-emergencies, your initial appointment may simply be a consultation how we can help you achieve your goals. When you decide that this is the place for you we will start with a comprehensive examination, xrays, photographs of your mouth and face, and a screening for gum disease and oral cancer. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require us to spend some time evaluating and planning. We might want you to return to hear our assessment and recommendations.

Please assist us by providing the following information at the time of your consultation:

Any available xrays.
A list of medications you are presently taking.
If you have medical or dental insurance, bring your information and a blank form if available. This will save time and allow us to help you process any claims.
Completed registration, medical history, and dental history forms. They may be downloaded at the top of this page.

IMPORTANT: A parent or guardian must accompany all children under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medications (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require pre-medication for dental visits.

X-Rays

If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken by us. Digital X-rays should be emailed to us for best quality.
